Calendar Request

Please have all information completed as to not delay your approval. This application must be completed and turned in a minimum of 10 weeks prior to the event.

All calendar requests must include the Event Budget Form.

  • Let us know if you any details that will help people understand what this event will be accomplishing. If you have any graphics that you would like to use please submit them below.
    If yes please upload below for approval.
  • Section 3: Service Coordination

    If you have any of these things already created please upload them below for approval.
  • Please explain what you would like announced, and if you have any specific requests. (i.e. you want to make the announcement, you have a skit to do for the announcement, you have a video)

    Please note there is a possible $35/hr. charge for a media person.
    If you have any other band members, please make a note in the Sanctuary Notes or Questions section.
    Please fill out the next section with how many of each you will need, and any special requests.
    If you have a PowerPoint presentation already created please upload the file below. If one has not been created yet, please submit the file at least one month prior to the event to insure that your presentation will work. If we do not have it one month prior to the event we make no guarentees that it will work.
  • Section 6: Childcare

    The cost of childcare is $7/hr. per provider. There will be a minimum of 2 providers watching children. Please take this into consideration when scheduling your event. You may want to include this cost into your event if necessary.
  • Event Budget

    Please fill the following blanks with the explanation of the funds needed followed by the desired fund total you wish to receive from Summit Church to make your event happen. Please make sure all fields are full.... even if it is $0.00. Please make sure to explain the breakdown of each catagory in the explanation box. YOUR EVENT WILL NOT BE APPROVED WITH OUT THE FOLLOWING INFORMATION!

  • Travel, Lodging, Food, Gift, Honorarium, Transportation
  • Decorations, Table Decorations, Table & Chair Rental

  • If you are going to use the sound system in the sanctuary, you are required to have a trained sound technician here at all times. The fee for that is $35/hr. Please figure out how many hours you will need someone here.
  • You are required to have a minimum or 2 childcare workers. Please keep in mind that you need to figure out how many childcare providers you will need to adequately serve the number of children that will be attending. A good ratio is 4 children per 1 worker. The fee is $7.00/hr per worker.
  • Will you provide scholarships to people that work the conference? Please estimate how much the church will pay in registration costs.
  • Please explain what you will charge for your event and where that money will be designated to.
  • Please give us a total cash budget for what you will need to make this event possible. Unfortunately this system will not add up your totals for you. Please double check to make sure you have the correct totals as to not delay the approval process.
